Subject to a clean proposal form, medical tests are waived off up to the age of 45 years. Above the age of 45, medical tests are mandatory and 50% of the test charges are reimbursed to the customer For entry age of up to 55 years, the maximum sum assured is Rs. 10 lakhs; for entry age of above 55 years, the maximum sum assured is Rs. 5 lakhs. Re and post hospitalization expenses are covered for 60 to 90 days respectively Pre-existing diseases are covered after four continuous policy renewals Hospital cash benefits are allowed under the Platinum plan only Ambulance charges are covered upto Rs. 15000 Coverage of 130 day care procedures Accompanying persons charges are covered up to Rs. 500 per day Patient care expenses are covered for those above 60 years; the amount is up to Rs. 350 per day A cumulative bonus is awarded for every claim-free year, from 10% to 50% Cost of health check up after four claim-free years is covered at authorized centers Additional cover may be purchased by the customer for accidental hospitalization in which the Sum Assured limit is enhanced by 25% up to a maximum of Rs. 1 lakh Upon completion of two policy years, there is coverage for cataract, tumour, hernia and similar ailments After the completion of 1 policy year, gallstones, kidney stones and similar conditions are covered Upon the completion of three policy years, there is coverage of Joint Replacement Surgery, that is required for reasons other than accident Eligibility for Future Health Suraksha Individual plan: Any Indian resident individual is between the ages of 18 to 70 years with lifetime renewal. Future Health Suraksha Family plan This is a health insurance plan that covers the hospitalization expenses of the individual and his family with the following features: One sum assured amount applies to all insured family members. Future Health Suraksha Personal Accident Insurance Plan: This is a policy that covers expenses arising out of any accident that results in the disability or death of the proposer. This is a smart insurance plan that protects both the insured person as well as his or her family members. The features are as follows: Cashless hospitalization at any of the companys 4200+ network of hospitals The following are covered: Permanent total disability Permanent partial disability Temporary total disability Death due to accident There is no medical examination required Hospital cash benefits are allowed under this plan There is an in-built cover for Repatriation Benefit and funeral expenses, in which an amount of up to a maximum of 1% of the Accidental Death Sum Insured is reimbursed for the repatriation of the insured persons remains. The limit of this is Rs. 12,500. Disability or death due to acts of terrorism is covered There is a loan protector benefit in which in case of permanent total disability or death, a lump sum amount is paid There is an Adaptation Allowance in which if there is Permanent Total Dismemberment or Disability, an amount of up to 10% of the principal sum insured will be reimbursed to the customer, towards modifying the house or vehicle. The maximum amount that will be paid is Rs. 50,000. Family Transportation Allowance is given when the insured person is placed in a hospital that is outside 100 Kms of his or her residence. The actual transportation expenses will be reimbursed up to a maximum of Rs. 50,000. Additional cover may be purchased by the customer for Accidental Medical Expenses, Life Support Benefit, Child Education Support and Accidental Hospitalization. Eligibility For Future Health Suraksha Personal Accident Insurance Plan: Any Indian resident individual is between the ages of 18 to 70 years with lifetime renewal. Children between the ages of 3 months and 25 years are covered as long as the parents are insured. Future Criticare Critical Illness Insurance plan: This is a health plan that has a primary focus on providing insurance benefits to people against any form of critical illness. The plan seeks to benefit the insured where hospital expenses and other medical related expenses are concerned, when the person suffers from a critical illness. Future Hospicash Hospital Cash Insurance Plan This is a policy that provides a cash benefit in case of hospitalization so that there is no drainage on the savings of the insured and his or her family. The features are as follows: There are four sub-categories of plans in this plan. They are as follows: Plan A and Plan B – there is no income criteria Plan C – income criteria of monthly income above Rs. 50,000 Plan D – income criteria of monthly income above Rs. 75,000 Under a single sum insured, the customer can insure himself as well as his or her family members (meaning the spouse and two children up to the age of 25 years) Subject to a clean proposal form, medical tests are waived off up to the age of 45 years. Above the age of 45, medical tests are mandatory and 50% of the test charges are reimbursed to the customer If the insured person or family member is hospitalized in the ICU within city limits, then the insured person will get twice of the per day benefit If the insured person or family member is hospitalized in the ICU within city limits, then the insured person will get thrice of the per day benefit There is an Additional Convalescence Benefit of Rs. 5000 if the hospitalization extends for more than 10 days The ICU Benefit is available for a maximum period of 10 days per hospitalization and a maximum of 20 days during the policy period The insured person does not need to have a prior mediclaim policy in order to avail of this policy plan Eligibility: Any Indian resident individual is between the ages of 18 to 65 years with lifetime renewal. Future Generali Health Surplus Top-up Plan: This is an additional policy that provides a larger cover that is more comprehensive. This is an ideal plan that can be used to cover hospitalization expenses that have incurred as a result of illnesses or accidents. The features are as follows: Pre and post hospitalization expenses are covered up 60 days and 90 days respectively Inpatient expenses are covered, such as room, nursing and board expenses for a minimum time period of 24 hours Additional inpatient expenses are covered such as medicines and drugs, diagnostic materials, x ray, operation theatre charges, specialists and consultant fees, etc. An individual may opt for this policy regardless of whether they have or dont have a mediclaim policy Joint replacement surgery is covered upon the completion of three policy years There is a family discount of twenty to sixty percent on family floater basis Subject to a clean proposal form, medical tests are waived off up to the age of 45 years. Exclusions or incidents that are not covered by Future Generali Health Insurance Plans The following conditions are not covered by the Future Generali Health Insurance Plans: The expenses that incur as a result of hospitalization as a result of diseases or injuries contracted within the first 30 days of the inception of the policy. This clause is not applicable when the policy is renewed at a later date.
